Diablo 4's endgame heavily focuses on Helltides, giving players strong enemies to fight while giving all sorts of rewards to make their characters stronger. Season 4 Loot Reborn in Diablo 4 gave Helltides a few new quality-of-life fixes while also centering the season around farming Helltides, with players spending most of their time farming in the Helltides.
One of the various tasks players can complete in a Helltide is completing the Accursed Ritual and summoning a powerful demon to fight. This battle will offer the highest challenge available within a Helltide but will reward players accordingly. This ritual can only be completed by using Baneful Hearts; here's where to get them.

Where To Get Baneful Hearts In Diablo 4
There are two ways for players to get Baneful Hearts: defeating Hellborne bosses or opening Tortured Chests. Opening Tortured Chests will require players to farm Aberrant Cinders, and not die as they take those cinders to the nearest chest to open it. Baneful Hearts aren't a guaranteed drop for chests, but their likelihood of dropping will increase in higher World Tiers in Diablo 4 and by opening more expensive chests like those that cost 275 Aberrant Cinders.
Hellborne bosses only spawn when players max out their Threat Level in a Helltide. Your Threat Level will increase with every enemy you defeat in a Helltide without dying, as dying will completely reset the bar displayed in the top right. While Hellborne enemies will spawn with each threshold passed on the Threat Level bar, only those that spawn when the bar is maxed out will drop Baneful Hearts. Baneful Hearts are also rewards for increasing Iron Wolves' Reputation, but the previous options are easier to farm.

How To Use Baneful Hearts In Diablo 4
Players can see how many Baneful Hearts they have whenever they are in a Helltide by looking at the top right of the screen, with Baneful Hearts being the icon to the right. Unlike Aberant Cinders, Baneful Hearts don't go away after a Helltide event ends and will stay forever until the player uses them. Baneful Hearts also carry over between characters, letting you farm them with one character while using them as another. They are saved to your account and won't need to be switched between characters using the stash.
Baneful Hearts are used during a Helltide in any World Tier. Look for the horned demon icon on the map within a Helltide to find the Accursed Ritual. One Baneful Heart must be placed on each of the three pedestals to summon the boss. Any number of players can contribute Baneful Hearts to summon the boss, as the activation of a pedestal is shared between all players in the area. After three Baneful Hearts are placed on the pedestals, the ritual will begin and enemies will begin spawning immediately, with the boss appearing soon after.
